HOW TO BOOK AN EXAM

All medical imaging exams require a requisition from a physician including screening exams like Whole Body MRI

Please have your doctor fax a requisition (We accept all health authority requisitions) to 778-613-2656. If you have a copy of the requisition, please send the requisition to imaging@elysianhealth.ca or you can upload when you click “book a scan” below.

We send the requisition to our Radiologists (specialist Doctors who read the images and make the reports) They protocol the exam detailing how to best scan based on the medical history. One of our booking clerks will call you within a couple days to book your exam.

If your doctor has not already provided you with a filled-out requisition form, please print the requisition below and have your doctor fill it out, and fax it to 778-613-2656 or email it to Elysian Imaging imaging@elysianhealth.ca
